About The Position

At Father Joe’s Villages, we are committed to helping individuals experiencing homelessness achieve stable housing and long-term self-sufficiency. The Transitional Bridge Shelter provides short-term shelter and intensive supportive services focused on rapidly connecting individuals to permanent housing and stability. The Singles Team supports adults in a structured shelter setting through trauma-informed, person-centered case management, housing navigation, and coordinated services that advance our mission to end homelessness one life at a time. As a Case Manager I, you play a key role in supporting individuals experiencing homelessness as they work toward housing stability and long-term self-sufficiency. You build trusting, empathetic relationships and provide individualized case management that removes barriers and connects clients to essential resources. Working as part of a multidisciplinary team, you help clients navigate complex systems, engage in supportive services, and make steady progress toward securing and maintaining permanent housing. Requirements

  • Bachelor’s degree in Social Work, Psychology, Sociology, Human Development, or a related field, or equivalent experience
  • Minimum one (1) year of experience working with underserved or vulnerable populations in a social services or related setting (internships, volunteer experience, or fieldwork welcome)
  • Skilled in trauma-informed care, motivational interviewing, and strengths-based case management
  • Knowledge of homelessness systems, housing interventions, eviction prevention, and crisis response strategies
  • Demonstrated ability to assess client needs and coordinate complex, multi-system service plans
  • Strong relationship-building skills with the ability to establish trust and maintain professional boundaries
  • Ability to remain composed and effective in high-demand, crisis, or emotionally complex situations
  • Strong organizational skills with the ability to manage multiple clients and priorities effectively
  • Knowledge of MS Office applications and case management systems, including HMIS
  • Demonstrated ability to maintain confidentiality and handle sensitive information appropriately
  • Strong written communication skills with an emphasis on clear, accurate, and timely documentation
  • Regular travel between worksites and client locations
  • A valid driver’s license and ability to meet organizational driving standards

Nice To Haves

  • Experience working in shelter, housing programs, or human services settings strongly preferred

Responsibilities

  • Conduct intake interviews and holistic client assessments to identify needs, strengths, and appropriate service pathways
  • Develop, implement, and monitor individualized case plans focused on housing stability, financial readiness, and long-term self-sufficiency
  • Coordinate referrals and advocate for access to housing, benefits, employment supports, and community-based services
  • Support clients in accessing Social Security benefits, disability programs, public assistance, and other income supports
  • Encourage engagement in medical care, behavioral health services, substance use treatment, and other supportive interventions
  • Maintain consistent client engagement to support progress, adjust goals, and respond to emerging needs or crises
  • Provide ongoing case management support, including crisis intervention, service coordination, and participation in multidisciplinary case conferencing
  • Maintain accurate and timely documentation in HMIS and case management systems, including progress notes and case records
