Requirements

  • Associate's degree in Human Services or related field with 2 years of case management experience, or equivalent combination of education and experience.
  • Proficiency in utilizing electronic database systems.
  • Strong proficiency in MS Outlook and MS Office.
  • Excellent customer service skills.
  • Effective oral and written communication skills.
  • Valid N.Y.S. Driver's License.
  • Registered and Insured Vehicle.
  • N.Y.S. Child Abuse Registry clearance.
  • Must meet all health standards for health department personnel as a subcontracting agency of the Onondaga County Health Department (OCHD).

Nice To Haves

  • Experience in facilitating educational groups with vulnerable populations.
  • Experience working with expectant mothers and parents of children under two years of age.

Responsibilities

  • Coordinating and implementing comprehensive case management services for pregnant women residing in the city of Syracuse.
  • Conducting assessments, developing service plans, monitoring services, and providing necessary follow-up.
  • Ensuring regular face-to-face contact with clients.
  • Maintaining confidentiality.
  • Offering screening services for domestic violence and perinatal depression.
  • Providing health education on various topics.
  • Facilitating prenatal care appointments.
  • Collaborating with relevant agencies.
  • Offering referrals to community resources.
  • Accurate documentation and maintaining case files.
  • Attending regular meetings with their supervisor for guidance, support, and training.
