Requirements

Associate's degree in Human Services or related field with 2 years of case management experience, or equivalent combination of education and experience.
Proficiency in utilizing electronic database systems.
Strong proficiency in MS Outlook and MS Office.
Excellent customer service skills.
Effective oral and written communication skills.
Valid N.Y.S. Driver's License.
Registered and Insured Vehicle.
N.Y.S. Child Abuse Registry clearance.
Must meet all health standards for health department personnel as a subcontracting agency of the Onondaga County Health Department (OCHD).

Nice To Haves

Experience in facilitating educational groups with vulnerable populations.
Experience working with expectant mothers and parents of children under two years of age.

Responsibilities

Coordinating and implementing comprehensive case management services for pregnant women residing in the city of Syracuse.
Conducting assessments, developing service plans, monitoring services, and providing necessary follow-up.
Ensuring regular face-to-face contact with clients.
Maintaining confidentiality.
Offering screening services for domestic violence and perinatal depression.
Providing health education on various topics.
Facilitating prenatal care appointments.
Collaborating with relevant agencies.
Offering referrals to community resources.
Accurate documentation and maintaining case files.
Attending regular meetings with their supervisor for guidance, support, and training.
