About The Position
Mount Carmel Guild Behavioral Health System, a CARF accredited division of Catholic Charities of the Archdiocese of Newark, is a leading provider of mental health and addiction services in Northern New Jersey. We are currently seeking a Full Time Case Manager for our Outpatient Department in Jersey City, NJ. Our Outpatient Program uses a multi-disciplinary approach; emphasis is placed on individualized assessment, goal-oriented treatment planning, periodic case review, and continuity of care for all persons served.
Requirements
- Requires bachelor's degree in a human services field
Responsibilities
- Function as part of OPD team
- Interfaces with OPD psychiatrist
- Assists staff psychiatrist with scribing clinical assessments
- Completes treatment plans and maintains staff psychiatrist caseload of clients
- Completes documentation according to agency and DMHAS standards
