This position is responsible for the overall coordination of care for individuals with developmental disabilities, with some responsibility for the overall and long-term elements of the program. The Case Manager Float will develop Individual Service Plans (ISPs) in collaboration with clients, families, funding agency representatives, and treatment team members. They will facilitate ISP/Individual Treatment Plan meetings and Interdisciplinary Team Meetings, ensuring timely completion of all necessary documentation, including progress summaries, progress notes, and monthly/quarterly updates. This role also serves as a primary contact for families, agencies, and other professionals, maintaining regular contact with clients and coordinating services among providers for continuity of care. The Case Manager Float will also be responsible for scheduling, attending, and transporting clients to appointments, advocating for client safety and welfare, and coordinating admission and discharge processes. They will communicate regularly with management to ensure treatment goals are met and provide necessary coverage in homes during emergency situations. Participation in required in-service training for continuing professional development and overall maintenance of clients' residential files are also key responsibilities.
