About The Position

The Case Manager Facilitator (Wraparound Facilitator) provides comprehensive case management, care coordination, and wraparound facilitation services for children and families enrolled in the Serious Emotional Disturbance (SED) Waiver Program. The primary purpose of this position is to ensure youth with complex behavioral health needs receive individualized, family-driven, community-based services that promote stability, independence, overall well-being, and success across home, school, community, and other life domains. The Case Manager Facilitator works collaboratively with youth, families, guardians, behavioral health providers, schools, foster care and child welfare agencies, insurance providers, and other community partners to develop and maintain effective, individualized plans of care. This position is responsible for coordinating services, facilitating treatment team meetings, monitoring progress, and ensuring SED Waiver services are provided and documented in accordance with applicable Medicaid, State of Kansas, and organizational requirements.

Requirements

Bachelor’s degree in human services, social services, psychology, sociology, behavioral health, or a related field preferred; or an equivalent combination of education and relevant human-services experience.
In the absence of a bachelor’s degree, a minimum of four years of relevant human-services experience is required.
Must be at least 21 years of age.
Experience working with children, adolescents, and families preferred.
Experience working with individuals diagnosed with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Substance Use Disorder (SUD) preferred.
Knowledge of community-based behavioral health services, case management principles, and family-centered care preferred.
Ability to coordinate services involving multiple providers, agencies, and support systems.
Strong organizational, prioritization, and time-management skills.
Ability to manage multiple deadlines, appointments, client needs, and documentation requirements.
Excellent verbal and written communication skills.
Strong interpersonal, problem-solving, conflict-resolution, and collaboration skills.
Ability to facilitate meetings involving clients, families, professionals, and community partners.
Ability to work independently while also functioning effectively as part of an interdisciplinary team.

Proficiency with computers, Microsoft Office applications, electronic health records, and other technology necessary to perform assigned duties.
Ability to learn and comply with Medicaid, SED Waiver, State of Kansas, and organizational documentation and service requirements.
Must successfully complete all required pre-employment screenings, including applicable background checks and drug screening.
Valid driver’s license with an acceptable driving record and ability to safely operate a company vehicle.

Responsibilities

Provide wraparound facilitation and case management services to children and families enrolled in the SED Waiver Program.
Coordinate individualized, strengths-based, family-driven services designed to address the youth’s behavioral health, educational, social, family, and community needs.
Collaborate closely with youth, parents, guardians, foster care agencies, schools, treatment providers, and other individuals identified by the family as part of the youth’s support system.
Promote family and youth involvement in identifying goals, strengths, needs, supports, and appropriate interventions.
Facilitate regularly scheduled wraparound and treatment team meetings.
Coordinate participation from appropriate team members, which may include mental health professionals, educators, foster care representatives, probation officers, medical providers, community agencies, family supports, and other service providers.
Encourage effective communication and collaboration among members of the treatment team.
Ensure meetings remain focused on the youth’s identified goals, strengths, needs, progress, and barriers to success.
Coordinate referrals and assist families through the initial SED Waiver enrollment process.
Educate youth, families, employees, treatment team members, and external partners regarding available SED Waiver services, eligibility requirements, program expectations, and applicable procedures.
Assist families with navigating behavioral health and community-based service systems.
Coordinate authorized waiver services based on identified client needs and approved plans of care.
Work closely with insurance and managed care providers to develop, coordinate, implement, and maintain individualized plans of care.
Ensure plans of care reflect the youth’s identified strengths, behavioral health needs, goals, supports, services, and desired outcomes.
Monitor implementation of plans of care and evaluate the effectiveness of services in achieving established goals.
Document significant changes in the youth’s circumstances, functioning, needs, or services and revise plans of care as appropriate.
Reconvene the wraparound team at least every 90 days, annually, and as otherwise required to review progress and update the plan of care.
Coordinate modifications to services when current interventions are no longer meeting the youth’s identified needs.
Maintain an assigned caseload while providing ongoing case management and SED Waiver services.
Maintain regular contact with youth, families, and treatment team members consistent with program requirements and individual needs.
Identify barriers to treatment and assist families with accessing appropriate behavioral health, medical, educational, social, financial, and community resources.
Advocate for youth and families when necessary to promote access to appropriate services and supports.
Support continuity of care during transitions between providers, schools, placements, levels of care, or other service systems.
Complete timely, accurate, and clinically appropriate documentation of services, interventions, meetings, contacts, progress, and client needs.
Maintain client records in accordance with Kanza policies, Medicaid requirements, SED Waiver requirements, and applicable State and federal regulations.
Ensure services are provided and documented in accordance with Medicaid billing and service-delivery guidelines.
Maintain client confidentiality and comply with HIPAA, organizational privacy policies, and professional standards.
Complete required assessments, care plans, authorizations, reports, and other program documentation within established timeframes.
Maintain effective working relationships with Kanza employees, families, schools, community organizations, healthcare providers, insurance providers, and other partnering agencies.
Communicate client needs, changes, concerns, and progress to appropriate treatment team members.
Participate in internal meetings, staffing meetings, trainings, and other activities as assigned.
Demonstrate professional communication, appropriate boundaries, and respect when working with clients, families, coworkers, and community partners.
Complete required SED Waiver, case management, crisis intervention, and other applicable training within 60 days of hire, as required by the State of Kansas and Kanza.
Maintain all required annual training, certifications, and competencies.
Remain knowledgeable regarding changes to SED Waiver services, Medicaid requirements, documentation standards, and community resources.
Travel throughout Kanza’s service area and to other locations as necessary to provide services, attend meetings, coordinate care, or meet organizational needs.
Safely operate a company vehicle when assigned or authorized.
Maintain a valid driver’s license and acceptable driving record consistent with Kanza requirements.
Perform other duties and responsibilities as assigned based on client, program, departmental, and organizational needs.
