About The Position

The Enhanced Step-Down (ESD) Specialty Critical Time Intervention (CTI) Case Manager provides specialized, time-limited case management and transitional support to adults with co-occurring Serious Mental Illness (SMI) and Intellectual and/or Developmental Disabilities (I/DD) who are transitioning from hospitals or emergency departments into the community. The position is performed in person in an office and in the community, with remote coordination and documentation also required. Travel within Nassau and Suffolk Counties and to partner hospitals, residential facilities, and community settings is required. The role requires a flexible schedule, including evenings and weekends, based on program needs.
