About The Position

The Case Manager/Discharge Planner is a member of a patient’s care team at The Center at Foresight, a skilled nursing facility serving patients who need physical rehabilitation and/or complex nursing care. The role assists in planning, developing, and coordinating patient care in accordance with applicable federal, state, and local standards, guidelines, and regulations to help maintain the highest quality of care. It also supports patient and family involvement and helps coordinate discharge planning and transitions from the Center.

Requirements

• At least one year of case management experience is preferred.
• Ability to make independent decisions, follow instructions, and accept constructive criticism.
• Leadership ability and willingness to work harmoniously with other personnel.
• Good judgment and ability to maintain highly confidential information.
• Knowledge of HIPAA compliance and requirements.
• Ability to plan, organize, develop, implement, and interpret departmental programs, goals, and objectives as they relate to the Company.
• Must pass a pre-employment background check and drug screen.

Responsibilities

• Meet with administration, medical and nursing staff, other departments, patients, and families when planning services.
• Partner with patients, family members, the patient’s care team, and social, health, and community agencies to support smooth transitions upon discharge from the Center.
• Ensure documentation is informative, descriptive, and consistent with the Center’s policies and procedures.
• Assist with the admission and discharge process.
• Attend care plan meetings and provide insight and professional guidance regarding patients’ needs.
