As a Case Management Department Supervisor, you'll play a key role in shaping and leading a team dedicated to delivering exceptional services and support. In this position, you'll oversee the growth, quality, and success of our case management program while coaching and developing talented case management professionals. You'll combine strategic leadership with hands-on support to ensure services are person-centered, effective, and compliant with state and federal requirements. Requirements

  • A Bachelor’s Degree in a human service-related field and two years of experience working with individuals with disabilities or aging
  • A minimum of three years of supervisory experience in case management services
  • Knowledge of the local community
  • Good oral and written communication skills
  • Knowledge in case management principles, procedures, and practices
  • Demonstrates adaptability to performing a variety of duties, occasionally changing from one task to another of a different nature without loss of efficiency or composure
  • Ability to negotiate and resolve conflict between dissenting parties
  • Ability to conduct detailed analytical evaluations and studies, and prepare related reports and recommendations
  • A valid driver’s license and proof of automobile insurance

Responsibilities

  • Lead, mentor, and develop a team of case managers, creating a positive and high-performing work environment.
  • Recruit, train, and retain top talent to ensure we have the resources needed to support current and future service demands.
  • Drive program development and continuous improvement initiatives that enhance service quality and outcomes.
  • Monitor performance, quality assurance standards, and compliance with regulatory and company requirements.
  • Partner closely with counties and community stakeholders to ensure contracted services meet all state and federal guidelines.
  • Oversee the delivery of comprehensive case management services, including assessment, planning, referral, coordination, monitoring, consultation, and advocacy.
  • Collaborate with organizational leadership on strategic planning, operational goals, budgeting, and service enhancements.
  • Build and maintain strong professional relationships with county partners, community agencies, and other key stakeholders.
  • Help promote and advance case management services through participation in committees, conferences, and industry organizations.
  • In partnership with the Director, develop, implement, and monitor policies, procedures, and best practices that support exceptional service delivery.
