About The Position

The Case Manager exercises independent judgment and professional discretion in the practice of case management to a select group of patients. The Case Manager collaborates with attending physicians and residents and serves as advocate to help patients and families navigate the acute episode of care. The Case Manager monitors patient’s continuing stay for resource utilization and helps prepare them for a safe and timely discharge. The Case Manager is proactive and uses collaborative problem solving skills to meet the needs of the patients and physician.

Requirements

• Bachelor's Degree in Nursing is required
• RN Licensure is required
• Minimum of 5+ Years of Position-Related Experience
• Patient care in the hospital setting
• Direct knowledge of care management
• Discharge planning
• Resource management
• Clinical expertise in area of responsibility and management of patients in that area
• Utilization management principals
• Excellent verbal and written communication skills
• Builds effective relationships with the healthcare team
• Demonstrates critical thinking skills
• Clinical and leadership skills
