About The Position

The Case Manager for Child & Adolescent mental health supports children, adolescents, and families by ensuring timely access to coordinated behavioral health services and resources. This role exists to promote stability, continuity of care, and measurable progress toward treatment goals. Key responsibilities include completing assessments, developing individualized service and treatment plans, coordinating with internal and community providers, delivering supportive counseling, and responding to urgent situations when needed.

Requirements

  • Bachelor’s degree in a human services field such as Social Work, Psychology, Sociology and Counseling
  • One (1) year of case management experience
  • Strong interpersonal skills
  • Empathy
  • Ability to work effectively with diverse populations

Responsibilities

  • Completing assessments
  • Developing individualized service and treatment plans
  • Coordinating with internal and community providers
  • Delivering supportive counseling
  • Responding to urgent situations when needed
  • Connecting with youth and caregivers to review progress
  • Updating plans as needs change
  • Confirming next steps with partner agencies and service providers
  • Returning calls
  • Arranging referrals
  • Ensuring information is shared appropriately
  • Providing timely support and helping stabilize immediate concerns when a situation escalates
  • Resetting the plan so services remain on track
