About The Position

PMHCC Case Management is a unit dedicated to providing recovery support services to individuals as they journey through their recovery process from the disease of addiction and other life challenges with the goal of achieving and maintaining long-term recovery. As case managers work with individuals in their recovery process, individuals are empowered to identify their strengths and talents and focus on the areas of need that he or she is willing to address at specific points in time. This collaborative approach is built upon the idea that all individuals, regardless of their challenges and walks of life, are people first.

  • Master’s Degree in social work, psychology or related field, with 3 years’ experience in social services, 1 of which includes supervisory experience preferred
  • Bachelor’s Degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, counseling, or education with 5 years of experience in social service field, 2 of which includes supervisory experience.
  • Or at least 1 year within the role of PMHCC-CM Team Leader.
  • Expertise in two or more of the following required: mental health, criminal justice system, substance abuse, homelessness.

Responsibilities

  • Manages, coordinates and directs activities of all staff on team in order to ensure necessary services are delivered to team’s caseload; directly supervises CM’s; provides direct service as may be required in emergency situations and to ensure compliance with standards relating to frequency of visitation.
  • Provides daily oversight/coordination to ensure a comprehensive approach to treatment; collaborates with external teams to ensure coordination between systems; provides linkage between DBH/IDS and CBH to minimize individual’s reliance on acute services.
  • Oversees and monitors individual’s assessment process; provides assistance in viewing individuals from a strength-based approach in preparing needs assessment and recovery-oriented service recovery plans, including training staff on the development of both.
  • Review daily inpatient alerts from webfocus to address and provide assistance to participants who present at Crisis Response Center and other medical facilities.
  • Access stakeholder’s electronic records and other platforms to provide effective services; understand participant treatment history to support the case manager in creating a comprehensive treatment plan from a strength-based approach. (i.e. webfocus)
  • Establishes networks and liaisons with high level community service providers to resolve problems and overcome barriers to service, including attending linkage and treatment team meetings as needed; provides advocacy mechanism to ensure that consumer needs are met.
  • Establishes a mechanism to engage designated treatment providers, including fostering their understanding of project goals and forming positive collaborative working relationships.
  • Conducts bi- weekly team meetings to provide group supervision; monitors staff daily schedules to ensure that tasks are evenly distributed and properly followed through; establishes a system of communication to ensure information flow and safety of staff and effective response when needed; maintains an information log of individuals discussed at each meeting.
  • Assigns caseload and team tasks to team members; provides conflict resolution and tactical suggestions in regard to disagreements to treatment approaches that arise within the team and other agencies.
  • Monitors the completion of and sign-off on all required documentation; trains and supervises staff in the completion of all paperwork; provides record of team productivity and outcome data as required; completes required monthly reports and other paperwork in timely manner.
  • Maintains compliance with all applicable substance abuse treatment standards as set by state and county funding authorities, and ensures that agency standards, policy and procedures are adhered to by team members.
  • Participates in Management, and other required agency and external meetings; attends in- service and other training in order to meet mandated training hours.
  • Plans and assists individuals to increase community tenure, enhance quality of life, and attain highest level of independent functioning.
  • Performs other duties as required by senior management
