About The Position
This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients in an established life management program. The role involves developing program plans/goals and evaluating client progress through mentoring and counseling sessions.
Requirements
- Bachelor's degree or greater from an accredited college or university in Social Work, Behavioral Science, or a related field.
- Three years progressively responsible experience providing direct case management social services.
- Valid Driver's License.

Responsibilities
- Conduct needs assessment to determine eligibility for program based on guidelines and requirements.
- Make referrals to other agencies if not appropriate for program.
- Prepares and maintains case records of assigned clients.
- Enters pertinent information into the Homeless Management Information System (HMIS).
- Establishes comprehensive goals for eligible clients.
- May accompany and/or provide transportation for clients to meetings related to case plan.
