About The Position

This position is responsible for arranging, advocating, coordinating, and monitoring the implementation, participation, and delivery of services designed (re-entry planning) to meet the specific needs of returning citizens.

Requirements

  • Two years of experience as a counselor, case manager, social worker in a criminal justice institution setting (or) in a program management and/or program development setting
  • Bachelor’s degree from an accredited college or university in social sciences, psychology, sociology, counseling, social work, criminal justice; OR An equivalent combination of related education, training and/or experience.
  • Must pass TB and drug screening
  • Must be on call and available for various shifts and weekend work.
  • Must be able to pass a federal background check.
  • Participate in annual in-service training and other training programs as required.
  • Knowledge of the methods and techniques of counseling and their application.
  • Knowledge of legal methodology and processing cases.
  • Ability to establish meaningful rapport with counselees.
  • Ability to write meaningful reports.
  • Ability to gather and interpret case histories.
  • Ability to work with various support agencies in exchange for information and data.
  • Ability to organize and supervise meaningful activities relevant to patient needs.

Responsibilities

  • Communicates and provides assistance in developing and implementing individualized re-entry/service plan(s) for assigned caseload.
  • Facilitates the acquisition of post-release services for returning citizen population.
  • Establishes, reestablishes, expands and strengthens relationships between clients and their families when appropriate.
  • Reviews offenders referred to the program and tracks progress through the case management process.
  • Conducts offender assessments to identify workforce and reentry barriers.
  • Implements criteria for identification of offender needs and develops and implements case plans to address removal of offender deficiencies.
  • Coordinates with appropriate agencies in obtaining necessary offender identification documents.
  • Assists in offender eligibility and application for various reentry benefits and services.
  • Works with and provides liaison with program staff, government agencies, offender families, faith-based support groups, and other organizations.
  • Conducts field duties and responsibilities such as home and agency visits, meetings, conferences, and workshops as required.
  • Maintains all required program documentation (reports, forms, statistics, etc.) for assigned caseload.
  • Develops and maintains records on participating offenders.
  • Documents case records and reviews reports.
  • Compiles and analyzes data and prepares summary reports.
  • Performs other duties as assigned or directed.
