About The Position

This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, complex, case management services for an assigned caseload of clients participating in an established life management program. The specialist will understand the uniqueness of the client’s history to determine the most effective program plans, develop comprehensive program plans/goals, and evaluate client's progress by conducting mentoring and counseling sessions. The role also involves serving as an advocate for the client to acquire services that will enable them to functionally cope with their environment and ensuring constant compliance with funding requirements.

Requirements

  • Bachelor’s degree from an accredited college or university in Social Work, Behavioral Science, or a related field AND Three years progressively responsible experience providing direct case management social services including assessing clients’ needs and developing individual, comprehensive, long-term actions plans for recovery utilizing a wide variety of resources OR Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
  • Ability to meet attendance requirements.
  • Ability to read, write, and communicate the English language.
  • Ability to communicate clearly and effectively both orally and in writing.
  • Ability to think clearly and quickly in order to maintain control of client caseload.
  • Ability to work multiple cases effectively and efficiently at the same time without becoming frustrated or disorganized.
  • Valid State Driver’s License
  • Requires a level 2 background screening.

Responsibilities

  • Interviews applicants to determine eligibility for program enrollment based on established facility guidelines and requirements.
  • Conducts needs assessment and obtains pertinent information.
  • Refers applicants to other agencies if not appropriate for the program.
  • Develops intake and case plans for clients.
  • Instructs clients in completing necessary paperwork including goals and/or budget forms.
  • Completes orientation check-off form and obtains release of information forms when appropriate.
  • Completes social history on new clients each week.
  • Meets regularly with clients to discuss and evaluate their progress, feelings, impressions, changes, and personal growth/development.
  • Prepares accurate and up-to-date records documenting client progress.
