About The Position

Valley Mountain Regional Center is seeking a dedicated Case Management Specialist to work alongside Service Coordinators, navigate complex legal and care systems, and help ensure that every consumer gets the support, and the chance to live a full life in their community. The Case Management Specialist manages a caseload and provides consultation and assistance to Service Coordinators to support consumers in remaining safely in the community and to prevent unnecessary admissions to State Developmental Centers (DCs). This role ensures that VMRC consumers receive legally mandated due process through the courts, are placed legally in DCs when required, and are successfully transitioned into preferred community living arrangements and community-based programs.

Requirements

  • Bachelor's degree in human services or related field and two (2) years of experience providing case management services; OR Bachelor’s degree in a non-human service-related field and three (3) years of experience providing training or direct services to individuals with developmental disabilities.

Responsibilities

  • Provide consultation and auxiliary services to Service Coordinators to prevent DC admissions and promote stable community placements, including intensive case management of high-risk cases.
  • Assess appropriate referrals to State Developmental Centers in collaboration with the Interdisciplinary Team (IDT).
  • Act as a liaison to Developmental Centers, Regional Projects, County Counsel, and District Attorney’s offices regarding DC admissions and discharges.
  • Monitor due process for DC admissions and discharges, including preparing petitions, completing court reports, renewing commitments (e.g., 6500, In re Hop), and representing VMRC in court proceedings.
  • Manage a caseload of DC consumers with emphasis on transitioning individuals into community placements and coordinating necessary support.
  • Manage a caseload of consumers at risk of DC admission.
  • Follow up on special incident reports submitted by Developmental Center or CCF staff.
  • Attend and/or conduct interdisciplinary team meetings for both community and DC consumers.
  • Coordinate development of Individual Program Plans (IPPs) and complete required case documentation.
  • Collect and report statistical data as requested for needs assessment and other purposes.
  • Participate in internal and external committees to strengthen services and partnerships.
  • Assist in training new Service Coordinators regarding admissions, discharges, and community resources.
  • Attend consumer services training as assigned.
  • Perform other related duties as assigned by the Program Manager.
