About The Position

We are seeking a Case Management Specialist Aftercare Residential to join our team. This remote hybrid role carries out case management services as required by the contract and licensing regulations. As a member of the Residential Aftercare Case Management team, you will work with 2 other team members and report to our Program Manager Aftercare.

Requirements

1-3 years of relevant experience
Bachelor's degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measures or therapeutic techniques, such as social work, psychology, sociology, guidance & counseling and child development, REQUIRED.
At least 21 years of age
Pass background check, physical, and drug screening
A valid driver’s license in the state you reside in
Proof of current vehicle insurance
Reliable transportation

Responsibilities

Exercise discretionary judgment in making decisions on the management and delivery of services to clients.
May attend various meetings and/or court hearings and facilitate groups as necessary.
Data entry into various software programs.
Crisis intervention.
Work with therapists as a team in coordinating services for clients and their families.
Complete Date Collection and Client Satisfaction.
Utilize community resources.
May be required to travel to attend meetings with assigned clients.
Other duties, as assigned.
