About The Position

Average hourly rate is $26.43 (including wage, incentives, bonuses, overtime, shift differential, etc.). Do you love helping children and families thrive? TFI Family Services is hiring a full-time Aftercare Case Manager to join our team! As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. You’ll focus on risk management and help families maintain stability during the aftercare period. You will provide crisis intervention and work with public and private agencies offering supportive services. You will maintain accurate documentation and reports. You will coordinate community and educational services for families. You will meet with families in-person to support reunification and resolve any concerns.

Requirements

• A bachelor’s degree in human services.
• A valid driver’s license.
• A willingness to learn—we’ll provide all the training you need!
• Predictive Index assessments assist hiring managers determine if a candidate may be a good match for the position.
