About The Position
Average hourly rate is $26.43 (including wage, incentives, bonuses, overtime, shift differential, etc.). Do you love helping children and families thrive? TFI Family Services is hiring a full-time Aftercare Case Manager to join our team! As an Aftercare Case Manager, you’ll support children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. We’re searching for someone who is compassionate and committed to strengthening families, can help families connect with the right community resources to maintain stability, is an advocate for the families and children we serve, and is eager to learn and grow in their skills to better support families and children. Requirements
- A bachelor’s degree in human services.
- A valid driver’s license.
- A willingness to learn—we’ll provide all the training you need!
- Predictive Index assessments assist hiring managers determine if a candidate may be a good match for the position.
Responsibilities
- Focus on risk management and help families maintain stability during the aftercare period.
- Provide crisis intervention and work with public and private agencies offering supportive services.
- Maintain accurate documentation and reports.
- Coordinate community and educational services for families.
- Meet with families in-person to support reunification and resolve any concerns.
