Aftercare Case Manager
About The Position
TFI Family Services is hiring a full-time Aftercare Case Manager to join their team. This role supports children and families who have achieved permanency through reintegration, adoption, custodianship/guardianship, or aging out of care. The Aftercare Case Manager focuses on risk management to help families maintain stability during the aftercare period, provides crisis intervention, and works with public and private agencies offering supportive services. Responsibilities include maintaining accurate documentation and reports, coordinating community and educational services for families, and meeting with families in-person to support reunification and resolve concerns.
Requirements
- A bachelor’s degree in human services
- A valid driver’s license

- Is an advocate for the families and children we serve.
- Is eager to learn and grow in their skills to better support families and children.
Responsibilities
- Focus on risk management and help families maintain stability during the aftercare period.
