About The Position

Abby Care is seeking a founding Case Management Coordinator to help launch and grow its presence in the greater Phoenix market. The role builds and owns relationships with health plan case management leaders, supervisors, and individual case managers, making referrals to Abby Care straightforward and ensuring partners receive timely updates. Initially, the position also supports provider relationships, including primary care offices, by helping with records, status updates, and stalled requests. The coordinator educates partners about program eligibility, referral steps, and what happens after a referral. They map case management organizations, identify team champions, arrange team sessions and in-person launch events, and keep case managers informed about referrals, families, and care plans. The role also manages authorization requests through submission, approval, and renewal, and records interactions, referrals, and authorizations in the CRM. This is a full-time hybrid opportunity based in the Phoenix area, with travel to meet partners across the market. The position suits a consultative, highly organized relationship-builder who is comfortable helping establish processes and playbooks as part of a founding team.
