About The Position

The Case Management Assistant is a pivotal role dedicated to streamlining workflow, facilitating communications, and enhancing the efficiency of the Case Management department. This role involves engaging with local community resources and contributing to a commitment to care that resonates with the needs and values of patients. The position plays a vital part in the orchestration of Case Management operations.

Requirements

  • High school diploma or equivalent preferred.
  • Proficiency in Microsoft Office products.
  • Strong typing skills with speed and accuracy.
  • Good visual acuity and communication skills.
  • Flexibility to work weekdays/weekends, evenings, or night shifts if necessary.
  • Availability during religious/legal holidays as scheduled.

Nice To Haves

  • Knowledge of medical terminology (preferred).
  • Experience with insurance/payor systems (preferred).

Responsibilities

  • Coordinate and facilitate Team Conference meetings.
  • Gather, take action on, and maintain Case Management documentation such as referrals, admissions, orders, and schedules.
  • Effectively coordinate communication of daily Case Management Assignment updates.
  • Complete pre- and post-discharge activities to ensure patients, families, and clinicians are aware and prepared.
  • Gather Quality Improvement and department Performance Improvement data.
  • Assist with collection of Key Care Indicators and clinical outcomes data.
  • Assist in regulatory and joint commission survey preparedness activities.
