Case Management Assistant II
About The Position
Coordinates and implements the transition of care (TOC) and discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care continuum through critical thinking, teamwork, and communication among care providers, patients, families, and external vendors to ensure timely discharge. This is a full-time, regular, non-exempt position at MMC-Memorial Medical Center. The position works day shifts, with variable days of the week, rotating weekends, 8-hour shifts, and 40 weekly hours. The pay range is $25.98 to $36.37 per hour. Eligible positions include a comprehensive benefits package.
Requirements
- High school diploma or GED.
- One year of recent relevant experience.
- Oral and written communication skills.
- Interpersonal and time-management skills.
- Ability to work effectively in a fast-paced environment with rapidly shifting priorities and competing demands.
- Ability to work independently with minimal direction.
- Ability to exercise discretion and prioritize tasks, seeking input as indicated.
- Intermediate PC and word-processing skills.
