About The Position

Community Action Coalition for South Central Wisconsin works to develop the economic and social capacities of individuals, families, and communities in Dane, Jefferson, and Waukesha Counties. The Case Coordinator develops relationships with participants to assess their needs and connect them to wraparound services, and develops processes for intake and assessment. The role uses motivational interviewing and other communication techniques to coordinate client intake, assessment, and access to CAC programs and services across three counties. Coordinators build community connections so people in need can access CAC programs and services, track and lead program outcomes across their service area, and oversee associated reporting and outcomes. The ideal candidate respects individual dignity and is committed to social justice and equity.
