About The Position

Trinity Shores of Port Lavaca is seeking a dedicated Caregiver to provide care and clinical-related services consistent with each resident’s comprehensive assessment and care plan. Caregivers are expected to deliver care with kindness, compassion, and joy while helping create a welcoming environment where residents feel at home, supported, and valued.

The position is offered on full- and part-time schedules, with a 6 a.m.–6 p.m. shift and required weekend availability. Prior experience providing personal care in a health care setting is preferred.

Responsibilities

• Provide residents’ daily and as-needed personal care, including grooming, bathing, dressing, and oral care.
• Change bed linens and maintain appropriate grooming supplies in residents’ rooms.
• Ambulate and transfer residents using appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other assigned information.
• Turn and position residents as necessary.
• Prepare residents’ rooms with necessary items such as blankets, pillows, medical equipment, and toiletries.
• Ensure a report has been given regarding assigned residents at the beginning of each shift.
• Respond promptly and courteously to call lights and buzzers, treating each light as an emergency until answered.
• Seek assistance from the charge nurse when unsure about an assignment, procedure, or situation.
• Provide care that protects and promotes residents’ rights, dignity, privacy, confidentiality, and active participation.
• Offer and respect residents’ choices, handle their property respectfully, and address them by their proper names unless they request otherwise.
• Observe and immediately report unusual occurrences; significant changes in residents’ physical or behavioral condition; decreased social interaction; angry or sad behaviors; and refusals of treatment to the Charge Nurse, ADON, or DON.
• Perform additional duties as assigned.
