About The Position

This position involves providing direct and indirect personal care to patients in their homes under the supervision of a registered nurse. The role requires utilizing safety measures, maintaining effective communication with supervisors regarding patient conditions, and following a written care plan developed by a healthcare professional. Responsibilities include documenting services provided, observing patients for changes in condition, and adhering to state regulations for Home Health Aides. The caregiver is not permitted to perform certain medical procedures such as changing sterile dressings, administering medication, or catheterizing patients, among others, unless specifically included in the plan of care.

Requirements

Adequate health status to perform described duties.
Ability to safely handle body fluids and hazardous waste products.
Must have the ability to effectively communicate with patients and staff.
Must meet personnel qualifications.

Nice To Haves

High School diploma or equivalent is preferred.
Graduated from an accredited Home Health Aide program preferred
Active HHA license preferred

Responsibilities

Provides direct and indirect personal care to the patient in the home setting under the direction of a registered nurse
Utilizes safety measures in the provision of care.
Maintains effective communication with supervisor regarding patient’s condition.
Follows written assignment to deliver patient services developed by an appropriate health care professional. (RN)
Documents and submits accurate accounts of services provided.
Observes the patient for changes in condition and behavior and report to the supervisor.
Submits required documentation in a timely manner.
Adheres to the role of the Home Health Aide in the home setting as defined in state regulation.
