About The Position

This position’s primary role is conducting initial assessments of caregivers’ needs and providing ongoing support to caregivers in compliance with the Executive Office of Aging & Independence (AGE) and agency guidelines. This position will also provide back-up support to the Home Care Department with initial intakes as needed.

Requirements

  • Experience with caregiver work or community out-reach will be considered.
  • Work with elder population, understanding family dynamics.
  • Excellent oral and written communications skills
  • Ability to effectively relate to a wide variety of personalities
  • Strong assessment skills
  • Basic computer skills
  • Demonstrated organizational and time management skills
  • Ability to multi-task and work in a fast-paced, high volume work environment.
  • Driver’s license and reliable transportation required.

Nice To Haves

  • Bachelor’s degree in human services or related discipline preferred

Responsibilities

  • Provides personal assistance in connecting caregivers with resources and services
  • Provides one-on-one counseling, coaching, and case-management
  • Conducts caregiver assessments and develops caregiver action plans
  • Facilitates family meetings
  • Maximizes service options available
  • Maintains a list of local resources including support groups, memory cafés, and gatherings
  • Connects caregivers to other community resources as needed (e.g., professional counseling)
  • Identifies, refers, and/or coordinates and facilitates caregiver training in group settings or for individuals
  • Utilizes evidence-based programs (e.g., Powerful Tools for Caregivers) and other programs viewed as successful for maximum benefit to caregivers.
  • Makes referrals to community support groups operated outside of the ASAP
  • Assists existing or new support groups with logistics, content, materials, etc.
  • Builds relationships with local service providers, employers, and community groups
  • Conducts caregiver informational sessions in the community, e.g., at Councils on Aging, public and private workplaces, etc.
  • Promotes caregiver outreach using a broad range of local contacts and media
  • Distributes caregiver information and materials to community locations, e.g., libraries, community centers, fairs, faith communities, etc.
  • Completes initial intakes based on regulatory guidelines.
  • Determines eligibility for programs.
  • Develop appropriate service plans that meet consumer choice and budget guidelines.
  • Partners with vendors to implement home care services.
  • Assists consumers with problem solving and makes referrals as needed.
  • Screens for public benefits and assistance with referrals and applications as needed.
  • Document phone calls, visits, and all activities in accordance with AGE and agency requirements.
  • Maintains caregiver and consumer records accurately and within the mandated timeframe.
  • Communicate with case management staff upon a case transfer.
