About The Position

The Care Specialist provides person-centered assistance to clients in developing life skills and personal goals. The role coordinates care with a multidisciplinary team, helps clients connect with community resources, and provides crisis intervention and prevention services to clients and their families. The position also supports clinical record development and maintenance, teaches activities of daily living, and coordinates with schools and other family service providers.

Requirements

Associate's degree required; two years of experience or two years of college coursework in a related field may substitute for the degree requirement.
Effective written, verbal, in-person, and virtual communication skills.
Customer service skills.
Time management skills.
Analytical skills.
Knowledge of community resources.

Nice To Haves

Two years of professional experience working directly with youth and families in child welfare, a school, childcare, criminal justice, behavioral health, or a related setting.

Responsibilities

Coordinate client care with a multidisciplinary team and develop and maintain clinical records, including treatment plans, progress notes, reports, and correspondence with external agents, in compliance with applicable standards.
Teach and train clients in activities of daily living in group and individual settings, comply with clinical documentation requirements, and provide services according to organizational guidelines and protocols.
Connect clients with appropriate community support systems and maintain regular communication with schools, OFCs, and other family service providers involved with the client and family.
Provide telephone and on-site crisis intervention and prevention, as well as follow-up, to clients and their families.
