About The Position

Coordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care. This role involves evaluating nursing and clinical intervention outcomes, monitoring resident medication usage, and participating in discharge planning. The Care Plan Facilitator will interview residents, review medical documentation, and collaborate with the healthcare team to assess resident needs and implement care plans. Proficiency in computer navigation and keyboarding is essential, as is participation in professional growth and development activities. The role also includes collaborating with assigned RNs to refer residents for specialized treatment.

Requirements

Associate’s Degree (AA) In nursing (ADN) Required
1-3 years Nursing home facility experience within the last three years Required
Registered Nurse Iowa Board of Nursing (IBON) (United States of America) Required
Basic Cardiac Life Support & First Aid - American Heart Association within 60 Days Required
Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required

Nice To Haves

Bachelor’s Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred
1-3 years Experience in the RAI process Preferred

Responsibilities

Completion of the MDS, CAAS and Care Plans by required completion dates and submit to the state of Iowa by submission date.
Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary.
Monitor resident medication usage and results.
Participates in discharge planning and other necessary team conferences.
Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions.
Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate.
Demonstrates accurate keyboarding proficiency and computer navigation skills.
Participate in activities aimed at professional growth and development including conferences or continuing education activities.
Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists.
Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement.
Resident care may be needed upon department needs.
