About The Position

Coordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care.

Requirements

  • Associate’s Degree (AA) In nursing (ADN) Required
  • 1-3 years Nursing home facility experience within the last three years Required
  • Registered Nurse Iowa Board of Nursing (IBON) (United States of America) Required
  • Basic Cardiac Life Support & First Aid - American Heart Association within 60 Days Required
  • Dependent Adult Abuse Mandatory Reporter - Iowa Department of Human Services within 180 Days Required
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation.
  • Being aware of others' reactions and understanding why they react as they do.
  • Actively looking for ways to help people.

Nice To Haves

  • Bachelor’s Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred
  • 1-3 years Experience in the RAI process Preferred
  • 1-3 years Experience with Microsoft Suite (Word, Excel, PowerPoint, Teams, and Outlook) Preferred

Responsibilities

  • Completion of the MDS, CAAS and Care Plans are done by required completion dates and submit to the state of Iowa by submission date.
  • Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary.
  • Monitor resident medication usage and results.
  • Participates in discharge planning and other necessary team conferences.
  • Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions.
  • Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate.
  • Demonstrates accurate keyboarding proficiency and computer navigation skills.
  • Participate in activities aimed at professional growth and development including conferences or continuing education activities.
  • Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists.
  • Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement.
  • Resident care may be needed upon department needs.
