About The Position

Care Partners provide assigned residents with routine daily nursing care and services in accordance with each resident’s assessment and care plan, and as directed by supervisors. The role also includes assisting residents with housekeeping, laundry, social needs, meal service, and cleanup; documenting care; communicating important information between shifts; and reporting changes in residents’ conditions. Care Partners must protect residents’ health information and keep their certification current through continuing education and training.

Requirements

High school education or equivalent.
Current state certification as a Certified Nurse Assistant (CNA).
Licensed Certified Nurse Assistant who has completed a state-approved training program and any required examinations in accordance with state law.
Able to read, write, speak, and understand English.
Ability to make independent decisions when circumstances warrant.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies or personnel, and the general public.
Patience, tact, a cheerful disposition, enthusiasm, and willingness to handle difficult residents.
Documentation of certification must be provided upon application.

Nice To Haves

Prior experience in assisted living (AL) or a skilled nursing unit (SNU).
At least one year of prior experience as a certified nurse aide.

Responsibilities

Complete the daily shift report and communicate urgent needs directly to the next shift.
Perform assigned tasks according to established policies and procedures and supervisor instructions.
Manage assigned tasks within the scheduled shift.
Assist with resident care, including housekeeping, laundry, social needs, and proper lifting techniques.
Assist with serving meals and essential cleanup.
Record entries on flow sheets, notes, charts, and other documentation in an informative and descriptive manner.
Report changes in a resident’s condition to the DON, Charge Nurse, or HWD as soon as practical.
Protect residents’ protected health information and promptly report suspected or known disclosure violations to the Administrator.
Keep certification current and participate in continuing education, staff meetings, in-services, and web-based training.
