About The Position

The Step Down Navigator is responsible for meeting all eligible patients to discuss the benefits of outpatient services as part of their continuum of care treatment. The Step Down Navigator will be responsible for communicating eligibility to key members of senior leadership and associated departments and will be responsible for rountinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes. The individual will have a thorough understanding of the Step-Down Playbook, a thorough understanding of the inpatient workflow process, as it pertains to case management and discharge planning. This individual can facilitate groups, assist with continued stay reviews and float to the inpatient units as needed. Requirements

Master's degree in Social Work, counseling, or a related mental health field from an accredited college or university.
LSW, LISW, LPC or LPCC issued by the State of Ohio.
Two years previous experience in a mental health or substance abuse treatment facility.
Direct experience with group therapy, crisis intervention, and treatment planning required.
Excellent verbal and written communication skills.
Diplomacy and discretion are essential.
Ability to prioritize multiple projects.
Professional demeanor and appearance required.
