About The Position

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and customer-focused Care Navigator in West Palm Beach. This full-time position supports the admissions process and helps ensure a seamless, empathetic experience for patients and families. The schedule is Monday through Thursday, 7:30 a.m. to 6:00 p.m.

Trustbridge Hospice is a community-based nonprofit that has cared for South Florida families since 1978. It provides hospice and palliative care, caregiver support, and bereavement services. The Care Navigator supports the organization’s patient-centered mission by coordinating referral intake and admissions, serving patients, families, and referral sources, and working with interdisciplinary team members.

Requirements

• Three years of experience in healthcare administration or customer service.
• Ability to handle a high call volume.
• Strong communication, organization, and computer skills.

Nice To Haves

• Associate degree preferred.
• Knowledge of hospice or home health programs preferred.
• Willingness to develop clinical understanding.
• Bilingual English/Spanish preferred.

Responsibilities

• Facilitate the conversion of referrals to admissions by completing the intake process.
• Provide exceptional, empathetic, and professional customer service to patients, families, and referral sources.
• Communicate effectively with interdisciplinary team members to support timely and accurate admissions.
• Maintain accurate records and documentation in accordance with organizational policies.
• Support a compassionate, patient-centered environment that reflects Trustbridge Hospice’s mission and values.
