Blackbird Health’s Care Navigators are an essential part of its integrated, collaborative care team. After a new patient or family begins care, the Care Navigator serves as a consistent source of support and takes ownership of their experience by answering questions, providing education, guiding them through next steps, and ensuring they are well cared for. The role is focused on providing responsive, empathetic, and professional support to established patients and families while coordinating administrative tasks that enable care. The Care Navigator is the main point of contact for patients, managing inbound calls, messages, and emails and following up transparently. The role guides families through Blackbird’s care model, including scheduling initial evaluations and ongoing therapy and medication-check appointments, and manages a group of providers to support appropriate scheduling and timely access to care. Other responsibilities include managing provider orders, screenings, and scheduling; supporting patients and providers in person; maintaining office operations; and protecting patient privacy in accordance with HIPAA and family consent laws. This position is onsite five days a week.
