About The Position
The Care Navigator serves as a vital liaison between inpatient treatment, outpatient services, community providers, patients, and families to ensure smooth transitions of care. This position focuses on patient engagement, discharge coordination, continuity of care, community outreach, and outpatient program growth. The Care Navigator assists patients diagnosed with Serious Emotional Disturbance (SED), Alcohol & Drug (A&D) disorders, and Adult Mental Health conditions while promoting ongoing treatment participation and reducing readmissions.
Requirements
- High school graduate or equivalent with one to three years of related work experience.
Nice To Haves
- Associates’s Degree in Mental Health related field and three to five years of experience in related work experience.
Responsibilities
