About The Position

The Stepdown Navigator serves as the primary liaison between the hospital, receiving providers, families, and external agencies. This role involves identifying and coordinating with appropriate step-down programs (RTC, PHP, IOP, and Outpatient) based on patient needs. The Stepdown Navigator will attend treatment team meetings to provide updates on discharge planning and placement status, offer emotional support and guidance throughout the transition, and educate patients and families on the step-down process and what to expect at the next level of care. Compass Intervention Center is a free-standing residential treatment center providing psychiatric services to male and female children and adolescents from ages 10 to 17, offering a full continuum of care including residential treatment, virtual programs, intensive outpatient, and partial hospitalization services. They are the only facility in Tennessee offering every level of care to adolescents.

Requirements

  • Master’s Degree in Counseling, Social Work, Psychology, or a related behavioral health field required.
  • Minimum of 2 years of experience working with adolescents in a mental health or behavioral health setting.
  • Excellent communication skills, both written and oral.
  • Strong knowledge of the behavioral health continuum of care and transition processes.

  • Licensure or license-eligible (e.g., LPC, LMSW, LMFT, etc.) preferred.

Responsibilities

  • Serve as the primary liaison between the hospital, receiving providers, families, and external agencies.

  • Attend treatment team meetings to provide updates on discharge planning and placement status.
  • Provide emotional support and guidance throughout the transition.
  • Educate patients and families on the step-down process and what to expect at the next level of care.
  • Work closely with the clinical team to assess each patient’s readiness and suitability for step-down care.
  • Develop individualized transition plans that align with clinical recommendations and family circumstances.
  • Ensure all clinical documentation, referrals, and authorizations are completed in a timely manner.
  • Address barriers to access, including insurance, transportation, and scheduling.
