About The Position

Spring Mountain Treatment Center is currently seeking a dynamic professional to join our team as a Care Navigator. This position is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. Communicates eligibility to key members of senior leadership and associated departments. Responsible for routine collection and data entry, specific to Outpatient eligibility into Midas for trending report purposes. Have thorough understanding of Step-Down Playbook, implementing components as needed and workflow process as it pertains to case management and discharge planning preferred. Presentation and customer service skills are key. They present good communication skills. ABOUT SPRING MOUNTAIN TREATMENT CENTER: Spring Mountain Behavioral Services has provided mental health services to the Las Vegas community since 2001, treating children, teens, adults and senior adults with behavioral health or addiction issues. We offer a supportive and healthy environment with 24-hour care that allows people of all ages to make positive changes in their lives. Our primary location, Spring Mountain Treatment Center, focuses on the stabilization of acute symptoms for patients age 5 to 65, along with select outpatient care programs for teens. Our secondary location, Spring Mountain Sahara, houses our Adult Outpatient and Senior Adult Inpatient programs. Spring Mountain Treatment Center is a 110-bed mental health facility that provides acute crisis stabilization services for adults, teens, and children in a secure inpatient setting. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. Requirements

  • Minimum two (2) years’ experience working in psychiatric/mental health field.
  • Possess knowledge of clinical issues related to substance abuse patients and their discharge planning.
  • Obtain Basic Life Support certification.
  • Ability to follow both written and verbal instructions.
  • Understand and honor the established hospital rules, regulations and Policy and Procedures.
  • Good communication skills, i.e., reading, writing, and speaking.
  • Good leadership qualities which includes self-control, patience, and empathy.
  • Must successfully pass background check (includes criminal history, Child Abuse and Neglect Registry, and ABI/FBI fingerprint check).

Nice To Haves

  • Thorough understanding of Step-Down Playbook, implementing components as needed and workflow process as it pertains to case management and discharge planning preferred.
  • Bachelor degree in mental health or related field preferred.
