About The Position

The Utah Department of Health & Human Services, Division of Correctional Health Services, is seeking a Care Navigator (Caseworker II) to join their team. This role involves providing evidence-based, individualized interventions to safely stabilize a wide range of mental health issues and help individuals maintain their best level of functioning while in custody. The agency's goal is to develop and carry out high-quality, evidence-based programs aimed at improving individual outcomes and enhancing community safety by implementing strategies, promoting accountability, and strengthening partnerships while maintaining a positive work environment. An offer of employment is contingent on passing a DHHS background check, pre-employment drug testing, and a Utah Department of Corrections (UDC) background screening.

Requirements

  • Licensure as a Social Service Worker or Certified Substance Use Disorder Counselor or working towards getting their licensure.
  • Experience working with substance use disorder and/or mental health treatment.
  • Experience teaching educational classes.
  • Experience in case management principles.
  • Effective speaking, reading, and written communication skills.
  • Knowledge of risk assessments and criminogenic risk factors.
  • Familiar with medication treatment for opioid use and other substance use disorders.
  • Must successfully pass a criminal history check.
  • Must pass pre-employment drug testing.
  • Must pass a Utah Department of Corrections (UDC) background screening.

  • Underfill as a certified case manager.
  • Hold current Licensure as a Social Service Worker or Certified Substance Use Disorder Counselor or are working towards getting their licensure.
