About The Position

Work remote on a consistent PST schedule with a capped caseload—focus on patients, not paperwork. This role involves making outbound calls, scheduling patient visits and procedures, coordinating care between providers and patients, gathering medical history, documenting in the EHR, following up after visits, escalating issues to the clinical team, and keeping patients engaged and supported. The goal is to improve access to brain health care.

Requirements

  • Comfortable making high-volume outbound calls daily
  • Basic medical terminology knowledge
  • Experience speaking with patients/customers by phone
  • Clear, professional communication skills
  • Able to adapt in a fast-changing environment
  • Structured work, clear targets, and talking to patients all day.
  • Can handle call volume and still stay empathetic.

Nice To Haves

  • Care coordination or patient navigator experience
  • Spanish language skills
  • Experience with cognitive/elder care populations

Responsibilities

  • Make 20-70 outbound calls/day; land ~10 patient check-ins
  • Schedule visits, labs, imaging, and outside referrals
  • Coordinate care between providers and patients (clear ownership)
  • Gather medical history and document in EHR
  • Follow up after visits with next steps
  • Escalate issues to clinical team (fast feedback)
  • Keep patients engaged and supported on calls
  • Own a panel of ~140 patients max.
