Requirements

• Master’s Degree in Nursing or related field required. Candidates who are actively pursuing their Master’s will be considered.
• Current registration and membership in good standing with the College of Nurses of Ontario (CNO).
• 3 years of recent experience in acute care and/or surgical and/or emergency nursing, preferably with clinical research and formal teaching experience.
• Knowledge in MS Office (Outlook, Word, Excel, PowerPoint, etc.).
• Knowledge in Orthopaedic care pathways, evidence based programs, and clinical practice standards, urology care pathways, remote care monitoring pathways.
• Understanding of evidence of program design and resources development skills, including knowledge of evaluation methodologies, quality improvement processes, data analysis, program development.
• Knowledgeable in College of Nurses of Ontario standards of practice, and Canadian Orthopaedic Nurses Association (CONA) standards.
• Possesses expert clinical skills in assessment, planning, implementing, evaluating and documenting the delivery and coordination of care for the surgical patients and families’ community resources.
• Excellent practice program development, project management and report writing skills.
• Excellent leadership, collaboration, decision making, problem recognition and problem solving, and critical thinking skills.
• Possesses time management and organization skills with ability to manage competing demands in a fast-paced, multi-site, multi-disciplinary health care environment.
• Demonstrate ongoing commitment to continuing professional development.
• Excellent attendance and discipline free record required.

Responsibilities

• Act as a member of a specialized interdisciplinary surgical care program in the Surgical & Perioperative programs to support assessment, follow-up, and treatment referral processes aligned with clinical best practice guidelines and evidence-based research.
• Remain abreast of new developments in surgical care related evidence-based programs to disseminate and/or utilize developments and new knowledge into clinical practice; integrate research into practice to ensure best practice standards are maintained within selected procedures and subspecialties.
• Participate in the implementation, monitoring, and evaluation of standards specific to surgical care assessment and health follow up.
• Conduct frequent environmental scans to identify gaps in clinical delivery for opportunities for improvement and promote ongoing quality assurance; and plan, implement, evaluate, and report on outcomes to all stakeholders, including mapping present and future trends, to create and sustain a seamless and continuous process for patients undergoing surgery.
• Influence clinical practice and patient outcomes and collaborate with the broader health care team and community partners regarding complex and challenging patient care situations.
• Participate in the collection and evaluation of health care data required that maybe required by any stakeholders, including the MOHLTC, OH and CCO.
• Develop and foster links with external partners to facilitate continuity of patient care.
• Supports the patient through the available treatment options to develop health care plans, and along the entire episode of care: prevention, diagnosis, preparation/plan, treatment, recovery/rehabilitation and monitoring/management to ensure that every patient receives care that is coordinated and navigated along the patient care path and/or entire process (preoperatively, postoperatively, recovery and rehabilitation).
• Liaise with patient primary care teams (e.g. physicians, sub-specialists, radiologists, surgeons, primary health practitioners, and community agencies) to facilitate the delivery of patient care services.
• Provide supportive care resources to patients and families to address psychosocial, practical, and informational needs across their entire continuum of care from pre-op to their post-acute phase.
• Develop and deliver to patients/families and staff surgical health educational sessions assessment, intervention and coordination within the care pathway.
• Develop and optimize remote care monitoring pathways in collaboration with the surgical interdisciplinary care team to improve target KPIs.
• Triage and support patient concerns and questions to stream patients into the right care post-operative care pathways.
• Other duties as assigned.
