About The Position

This position is 100% remote and is located within the Colorado Department of Human Services (CDHS), Civil and Forensic Mental Health Services (CFMH). CFMH provides a continuum of mental healthcare services for civilly and justice-involved individuals. The Care Navigation Services (CNS) team was created to address changes in Colorado’s statutory framework governing competency to proceed in criminal cases, transitioning defendants from the criminal justice system to the civil judicial system. The mission of the CNS team is to provide supervision and care coordination for individuals who have been formally charged with a felonious crime of violence or unlawful sexual behavior and who have been found by a presiding court to be both Incompetent to Proceed to adjudication (ITP) and Not Restorable to competency within the foreseeable future (ITP-NR). The team works within community, criminal justice, and secure settings, coordinating placement, ensuring accountability for patient care, providing communications with judicial stakeholders, and advocating for least-restrictive settings. Team members are required to maintain current skills and knowledge through continuing education and clinical supervision. Positions within the Care Navigation section may be required to travel throughout Colorado. These working arrangements are subject to change.

Requirements

Eight (8) years of experience in implementation and clinical oversight of behavioral healthcare programs in a supervisory role.
Experience in staff supervision.
Experience in clinical supervision.
Experience in clinical team leadership.
Experience in behavioral health crisis management.
Experience in policy and/or program implementation.
Experience in collaboration with other teams and leadership.
Experience ensuring service quality and that the needs of clients/patients, community, and referral sources are met.
Experience providing training, onboarding, oversight, and review of employees.
Experience providing mental healthcare services to individuals within a variety of related environments (e.g., inpatient, residential, detention, and/or forensic facilities).
Experience ensuring professional communication, documentation, and representation to both internal and external stakeholders within a variety of contexts including forensic and judicial.
Current and valid Colorado licensure as a LPC, LCSW, LMFT, LAC, or Licensed Psychologist.
State of Colorado residency at the time of application and throughout employment.
Ability to pass a full background check and reference check.
TB testing and flu vaccinations are required, as well as any other vaccinations or testing required by CDHS.

Responsibilities

Implements ongoing and routine auditing of each Care Navigator to ensure consistent process following, confidentiality, accurate data entry, appropriate placement recommendations, complete judicial reporting, and professional communication.
Confirms client assignment and monitoring by a Care Navigator throughout the placement process and ensures appropriate information is provided for continuity of care.
May attend in-person meetings with stakeholders and may be responsible for case coverage, potentially requiring travel throughout Colorado.
Identifies and reports process impediments to the Care Navigation Services Director, creates solutions, audits results, and responds to information requests.
Reports data management system problems to the data management team.
May be required to attend court proceedings and/or visit clients in custody or in placements to answer stakeholder questions.
Engages in site visits, stakeholder consultation, and feedback from individuals court-ordered to placements to ensure high quality and consistent practice implementation, monitoring, and auditing.
Provides direct supervision for Care Navigators, including oversight of processes, documentation, and legal submissions in partnership with the director.
Conducts individual and group supervision, training, onboarding, professional development, staff scheduling, and performance reviews.
Participates in the hiring process, including recruitment, interviewing, and evaluating fit.
Facilitates a sufficient level of position-related competencies and provides consultation and guidance to ensure supervisees function within statute, policy, and procedure.
May participate in and contribute to department trainings and review statutes.
Determines and contributes to staff coverage, potentially providing coverage for Care Navigator cases due to strict statutory timeframes.
May be required to travel to county jails and placement sites to observe clients and report back to the director with limited notice.
Consistently documents client contacts, collateral information, and case updates within the electronic health record system in a timely, thorough, professional, and accurate manner.
Responsible for legal documentation submitted to the courts, including recommendations for placements and treatment, opinions on modifying court-ordered conditions, annual reports, and termination reports.
Reviews and approves documents completed by supervisees for quality assurance.
Serves as a primary point of contact for judges, attorneys, guardians, and others affiliated with the judicial system, fielding questions and concerns via various communication methods.
May provide testimony when required on behalf of Care Navigation Services.
May be required to be in contact with law enforcement, maintaining professional boundaries and representing the department professionally.
