The Care Navigation Director serves as a key liaison between referral sources, healthcare providers, and Vetter communities. This individual oversees referral management processes, evaluates prospective admissions, and works closely with leadership teams to facilitate successful care transitions. Through clinical expertise, technology, and relationship-building, this role helps ensure patients are placed in the setting that best meets their needs.

Requirements

  • Active nursing licensure, with RN credentials preferred.
  • Three or more years of experience in senior care, post-acute healthcare, care transitions, admissions, or related leadership roles.
  • Working knowledge of EMR systems, referral management technology, CRM platforms, and healthcare software applications.
  • Strong clinical assessment skills and sound decision-making abilities.
  • Excellent communication, collaboration, and organizational strengths.
  • Ability to balance multiple priorities while maintaining attention to detail.
  • Willingness to travel throughout Nebraska as needed.

Responsibilities

  • Lead and coordinate care navigation activities for multiple Vetter Senior Living locations.
  • Monitor and manage incoming referrals through electronic referral platforms and admissions systems.
  • Review clinical information and other relevant factors to determine appropriate placement options.
  • Collaborate with hospitals, physician groups, discharge teams, and case management professionals to support efficient transitions into care.
  • Provide guidance to admissions personnel on referral review processes, response management, and best practices.
  • Maintain accurate records and communication within referral software and electronic medical record systems.
  • Partner with community leaders to align admission decisions with resident needs, clinical resources, and occupancy objectives.
  • Evaluate referral trends and performance metrics to identify opportunities for improvement and growth.
  • Foster strong partnerships with healthcare organizations by delivering exceptional customer service and responsiveness.
  • Travel between designated markets to support teams, strengthen referral relationships, and promote care navigation initiatives.
