Care Manager - Rock County, WI
About The Position
Community Care is hiring a Care Manager to join its Family Care Program team. The role supports the organization’s mission to help older adults and adults with physical and intellectual disabilities live as independently as possible in the community. The position is based in Walworth and serves Rock County, Wisconsin, with hybrid work available. This is a full-time opportunity with a weekday schedule and a $5,000 sign-on bonus. The Care Manager works collaboratively with an RN Care Manager and community partners to support program members, coordinate services, and develop care plans responsive to members’ health and social needs. Regular local travel is required, using a personal vehicle. Community Care is a Wisconsin-based local nonprofit with more than 40 years of experience. It coordinates and delivers supportive services to more than 13,000 Wisconsin residents and has local offices supporting the 23 counties it serves.
Requirements
- A bachelor’s degree in human services or a closely related area is required. A minimum of three years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
- Eligibility for long-term care functional screener certification is required.
- Regular local travel is required, along with use of a personal vehicle, a valid driver’s license, and proof of required insurance coverage.
Nice To Haves
- One year of experience working with older adults or people with physical and/or intellectual disabilities in a social service capacity is preferred.
Responsibilities
- Manage an assigned caseload of program members in collaboration with an RN Care Manager.
- Complete comprehensive member assessments, member-centered care plans, and functional screens.
- Provide ongoing care coordination and quality assurance monitoring of services.
- Maintain required documentation in accordance with established standards.
