About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to join our team in Carrick, PA. Since 2005, Crossroads has been a leader in treating patients with opioid use disorder, offering accessible, high-quality, evidence-based medication-assisted treatment (MAT). Our comprehensive approach aims to combat the opioid epidemic and guide individuals toward recovery. We operate outpatient, office-based clinics across the U.S., serving over 26,500 patients. Requirements

At minimum, High School Diploma or GED required.
Knowledge of compliant documentation practices.

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.

Responsibilities

Connect with patients monthly to address any needs and provide the resources to fulfill those needs.
Connect and maintain relationships with community partners for patient resources.
Work closely with a dedicated team to provide exceptional patient care.
Participate in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
