About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to join our team in Carrick, PA. We are a leading provider of medication-assisted treatment (MAT) for opioid use disorder, committed to offering accessible, high-quality, evidence-based care. Our outpatient clinics serve over 26,500 patients across multiple states, and we are passionate about making a difference in the communities we serve. As a Care Manager, you will play a crucial role in connecting patients with essential resources, fostering community partnerships, and contributing to our mission of combating the opioid epidemic.

Requirements

At minimum, High School Diploma or GED required.
Knowledge of compliant documentation practices

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.

Responsibilities

Connect with patients monthly to address any needs and provide the resources to fulfill those needs.
Connect and maintain relationships with community partners for patient resources.
Work closely with a dedicated team to provide exceptional patient care.
Participate in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
