About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to serve patients in the Cannonsburg and Bridgeville, PA areas. Since 2005, Crossroads has been a leader in treating opioid use disorder, offering accessible, high-quality, evidence-based medication-assisted treatment (MAT). Our comprehensive approach aims to combat the opioid epidemic and guide individuals toward recovery. We operate outpatient, office-based clinics across multiple states, including Pennsylvania, and are committed to improving access to treatment for thousands of patients. Requirements

High School Diploma or GED required.
Knowledge of compliant documentation practices.

Nice To Haves

Experience in a fast-paced environment servicing many clients is preferred.
Experience in healthcare, addiction medicine, community health, or care coordination is preferred.
Experience with building individualized care plans is a plus.
Lived addiction experience is a plus.

Responsibilities

Maintain relationships with community partners for patient resources.
Collaborate with a dedicated team to provide exceptional patient care.
Participate in community awareness and engagement events to promote Crossroads and its mission.
Build individualized care plans.
