About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to join our team. Since 2005, Crossroads has been a leader in treating patients with opioid use disorder, offering accessible, high-quality, evidence-based medication-assisted treatment (MAT). Our comprehensive approach aims to combat the opioid epidemic and guide individuals toward long-term recovery. We operate outpatient, office-based clinics across multiple states and are committed to diversity and inclusion. As a Care Manager, you will play a crucial role in connecting patients with essential resources and fostering community partnerships to support their recovery journey.

Requirements

High School Diploma or GED required.
Knowledge of compliant documentation practices

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.

Responsibilities

Connect with patients monthly to address any needs and provide the resources to fulfill those needs.
Connect and maintain relationships with community partners for patient resources.
Work closely with a dedicated team to provide exceptional patient care.
Participate in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
