About The Position

Crossroads Treatment Centers is seeking a dedicated Care Manager to join our team and help combat the opioid epidemic. Since 2005, Crossroads has been a leader in providing accessible, high-quality, evidence-based medication-assisted treatment (MAT) to individuals struggling with opioid use disorder. Our comprehensive outpatient, office-based approach is the gold standard in care, proven to reduce overdose deaths and foster long-term recovery. We are committed to expanding access to treatment across the U.S., serving over 26,500 patients in clinics located in Georgia, Kentucky, New Jersey, North and South Carolina, Pennsylvania, Tennessee, Texas, and Virginia. Requirements

At minimum, High School Diploma or GED required.
Knowledge of compliant documentation practices

Nice To Haves

Experience in a fast-paced environment, servicing many clients, is preferred.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Experience with building individualized care plans is a plus.
Lived Addiction experience is a plus.

Responsibilities

Connect with patients monthly to address any needs and provide the resources to fulfill those needs.
Connect and maintain relationships with community partners for patient resources.
Work closely with a dedicated team to provide exceptional patient care.
Participate in community awareness/engagement events to promote Crossroads, our mission, our principles, and our approach to addiction treatment.
