This role is for a Care Manager II in Colorado, covering territories in Denver Metro, Boulder Metro, and Douglas Metro regions. The position is field-based and involves managing new service plans, conducting supervisory visits quarterly, and renewing service plans. Structured family caregiving is a model where a patient lives with a principal caregiver who provides daily support, aiming to foster independence and offer care options as needs evolve. This service provides an alternative to nursing facility-level care while preserving patient dignity and privacy. The Care Manager II will be crucial to operations in Colorado, responsible for orientations, patient assessments, and caregiver training.

Requirements

  • 2+ years of experience working with frail or elderly disabled adults and caregivers.
  • Valid driver’s license with an insured vehicle for travel within the state.
  • Excellent verbal and written communication, organizational and time management skills.
  • Strong critical thinking and problem-solving skills.
  • Proficient in Microsoft (i.e., Word, Excel, Outlook).
  • Excellent training skills.
  • Team-player mentality.

Nice To Haves

  • Previous field experience in a medical or case management environment.
  • Experience with data input, case review/case note entry; Salesforce experience is a plus.
  • Hands-on experience in home care.
  • Working knowledge of ADL/ IADL assessments.

Responsibilities

  • Performing orientations, including signing of paperwork.
  • Conducting initial assessments during orientations.
  • Performing periodic supervisory visits/check-ins for on-care cases.
  • Training caregivers/patients as required per case.
  • Interacting with the caregiver and other stakeholders as needed.
  • Engaging in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborating in care practice and communicating case impressions to appropriate staff.
  • Critically assessing procedures and implementing improvements for case evaluation, feedback, and caregiver training.
  • Wearing multiple hats as operations develop.
  • Traveling within the state of Colorado.
