About The Position

The Care Manager (CNA) assists residents with daily living needs according to their care plans. Duties include personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. The role is per-diem on the second shift, from 2:30 p.m. to 10:30 p.m., and pays up to $22 per hour. The Care Manager reports to the Health & Wellness Director.

Requirements

• Certified Nursing Assistant or Certified Home Health Aid.
• Knowledge and experience working with the elderly population.

Nice To Haves

• Dementia experience is preferred.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
• Maintain residents’ safety, comfort, dignity, and confidentiality.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
