About The Position

The Care Manager (CNA) assists residents with aspects of daily living as indicated in each resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. This is a per-diem, hourly, non-exempt role scheduled for the second shift, 2:30 p.m. to 10:30 p.m., at $22.00 per hour. The Care Manager reports to the Health & Wellness Director.

Requirements

• Certified Nursing Assistant (CNA) or Certified Home Health Aid.
• Ability to perform the described physical activities, including lifting or carrying up to 50 pounds and intermittently lifting and supporting residents.
• Must use proper body mechanics.

Nice To Haves

• Knowledge and experience working with the elderly population.
• Dementia experience preferred.

Responsibilities

• Provide resident care, including bathing, dressing, toileting, and feeding, as outlined in care plans.
• Maintain residents’ safety, comfort, dignity, and confidentiality.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts.
• Document all services provided to assigned residents during the shift.
• Immediately report any change in a resident’s condition to a supervisor.
• Assist residents with daily-life needs as indicated in their care plans, including food service, housekeeping, laundry, behavior management, socialization, and activities.
